How it works

Pramlintide is a synthetic analogue of amylin, a hormone co-secreted with insulin by pancreatic beta cells. Three proline substitutions make it stable and non-aggregating, unlike native human amylin. It acts on amylin receptors (calcitonin receptors paired with receptor activity-modifying proteins) in the area postrema and other brain regions, slowing gastric emptying, suppressing the post-meal release of glucagon and promoting satiety. Together these effects lower blood glucose rises after meals. Pramlintide is FDA-approved (as Symlin, first approved in 2005) for adults with type 1 or type 2 diabetes who use mealtime insulin and have not reached glucose targets with insulin alone. In controlled trials it modestly lowered HbA1c and reduced post-meal glucose spikes, and was associated with small weight loss rather than the weight gain often seen with insulin intensification. Its most important risk is severe hypoglycaemia when combined with insulin, particularly in type 1 diabetes, which carries a boxed warning; mealtime insulin doses are typically reduced when it is started. Amylin-based approaches have renewed interest for obesity, with longer-acting analogues such as cagrilintide being studied, but pramlintide itself is not approved for weight loss.
