How it works
Vasoactive intestinal peptide (VIP) is a 28-amino-acid neuropeptide found in nerves throughout the brain, gut, lungs, heart and blood vessels. It binds VPAC1 and VPAC2 receptors (and with lower affinity PAC1), raising intracellular cAMP. This relaxes vascular and airway smooth muscle, causing vasodilation and bronchodilation, stimulates water and electrolyte secretion in the intestine, and modulates immune cells toward a less inflammatory profile. It also acts in the brain's circadian clock in the suprachiasmatic nucleus. VIP has a very short half-life in blood, which limits systemic use. A synthetic form, aviptadil, combined with phentolamine is approved as an intracavernosal injection for erectile dysfunction in some European countries. Intravenous and inhaled aviptadil were studied for respiratory failure in COVID-19 and pulmonary hypertension, without achieving US approval. Intranasal VIP has been used in some clinical practices for chronic inflammatory response syndrome, but support comes from small uncontrolled case series. Excess VIP from rare tumours (VIPomas) causes severe watery diarrhoea, illustrating its potent effects. The record lists subcutaneous administration, but its dosing describes intranasal spray.
