Quick start
- Reconstitute: Add 2.0 mL bacteriostatic water to the 10 mg vial → 5 mg/mL
- Typical dose: 4 mg once daily (SC) for 28 days

How it works
ARA-290 (cibinetide) is an 11-amino-acid peptide designed from the helix B surface of erythropoietin (EPO). Unlike EPO, it does not bind the classic EPO receptor homodimer that drives red blood cell production. Instead it selectively activates the innate repair receptor, a heteromer of the EPO receptor and the beta common receptor (CD131), which is upregulated in tissues under stress or injury. Activation of the innate repair receptor dampens pro-inflammatory cytokine signaling, reduces apoptosis and supports tissue protection and repair. In rodent models, ARA-290 reduced neuropathic pain, protected nerves and promoted regrowth of small nerve fibers, and showed protective effects in models of kidney, heart and retinal injury. Because it lacks erythropoietic activity, it avoids the clotting and blood-pressure risks associated with EPO. ARA-290 has been studied in small phase 2 human trials. In patients with sarcoidosis-associated small fiber neuropathy and in people with type 2 diabetes and neuropathy, 28 days of daily subcutaneous dosing was associated with improvements in neuropathic symptoms, corneal nerve fiber density and, in diabetes, some metabolic markers. These trials were small, and the compound is not approved for any indication.
Reconstitution steps
- Draw 2.0 mL bacteriostatic water with a sterile syringe
- Inject slowly down the vial wall; avoid foaming
- Gently swirl or roll until fully dissolved (do not shake)
- Label with reconstitution date and refrigerate at 2–8°C, protected from light
