How it works
Semax is a synthetic heptapeptide (Met-Glu-His-Phe-Pro-Gly-Pro) derived from the ACTH(4-10) fragment of adrenocorticotropic hormone, with a Pro-Gly-Pro tail added for stability. It retains the brain-active properties of this ACTH fragment without meaningful hormonal (cortisol-releasing) activity. In rodent studies, Semax rapidly increases brain-derived neurotrophic factor (BDNF) and its receptor TrkB in the hippocampus and cortex, influences dopamine and serotonin systems, and alters expression of genes involved in inflammation and blood vessel function after ischemic injury. It is thought to interact with melanocortin receptors, though the exact targets are not fully mapped. Standard Semax is approved in Russia and some neighboring countries as an intranasal medication for stroke recovery and certain cognitive and neurological conditions. Russian clinical studies, generally small and not widely replicated outside the region, reported improved recovery after stroke and better attention and memory performance. N-Acetyl Semax (often sold as N-Acetyl Semax amidate) adds an acetyl group to the N-terminus, and often an amide at the C-terminus, to slow enzymatic breakdown. This version has not been tested in published controlled human trials, so claims of a longer half-life and stronger effect are inferred from chemistry and anecdote rather than clinical data.
