How it works
Semax is a synthetic heptapeptide built from the ACTH(4-7) fragment with a Pro-Gly-Pro tail added to slow enzymatic breakdown. Unlike full-length ACTH it has essentially no effect on cortisol release. Its actions are instead attributed to central nervous system signalling: in rodent studies it rapidly increases expression of brain-derived neurotrophic factor (BDNF) and its receptor TrkB in the hippocampus and cortex, and it modulates dopaminergic and serotonergic activity. Weak interaction with melanocortin receptors has been proposed but is not well characterised. Downstream, the BDNF and gene-expression changes seen in animal models are linked to improved learning performance, reduced neuronal damage after experimental ischemia, and anti-inflammatory effects in brain tissue. Semax is registered as a medicine in Russia, where it is used as nasal drops in stroke recovery and some cognitive disorders. Human evidence comes mostly from smaller Russian clinical studies, many of them open-label, in stroke and cognitive impairment. Large randomised trials meeting Western regulatory standards have not been published, and claims about focus or ADHD rest on limited data. Clinical use has been intranasal; the subcutaneous route listed in this record is less documented.
Reconstitution steps
- Draw 3.0 mL bacteriostatic water with a sterile syringe
- Inject slowly down the vial wall; avoid foaming
- Gently swirl/roll until dissolved (do not shake)
- Label and refrigerate at 2–8°C, protected from light
